Please print and complete the FIRST 3 FORMS:  "New Patient Info", "Privacy & Policies Signature Page", and "New Patient Med List", then bring them to your appointment.  

The "Office Policies" and "ZMA Privacy Policy" are for your information. (You do not need to bring those with you to your appointment.) 

If you have been seen by another Sleep Physician or Pulmonologist, please print and send the signed Records Release Form to their office as soon as you have scheduled with us so we can receive your records prior to your appointment.
